
What to Expect & Care Planning
Our Senior Carers will make an appointment to come and see you. They will carry out a needs assessment, risk assessments and a manual handling assessment to ensure you get the care you would like and to protect you and the carers’ health and safety.
We will develop a Care Plan with you and any relatives, friends or Social Worker you want to involve, which will reflect your preferences as well as needs. The Care Plan will be reassessed at regular intervals in order to ensure that the support delivered continues to meet your needs and preferences.
A day-to-day record as well as Care Plan is kept in your house.
A Senior Care Assistant will make an appointment to come and see you regularly to talk to you and any relatives or friends you would like to include, about the quality of care you receive and how happy you are with the service. We are committed to doing what you want us to do in the way you want it done.
You will have a named key worker who will take a particular interest in you, the service you receive and your carers. Our Senior Care Assistant will check at the beginning that everything is as you want it and then keep in regular contact with you and your carers. They are there to help you get the best care & support.

Arrangements through a Social Worker – opening a Care Account
Everyone is entitled to an assessment of needs from a Social Worker. If you have eligible needs you can have a Care Account with Social Services. For further information on opening a Care Account contact Somerset Social Services on 0845 345133.
If you are eligible you will be offered a Direct Payment, Personal Budget or Individual Service Fund.
Once the Social Worker has agreed with you what is available to you and what help and support you need one of our Senior Care Assistants will discuss and agree with you how to help you achieve your lifestyle within the budget you have been given, what you would like done and when you want your help and support.
